Parents, please check out these reminders from our nurses as we start the new school year! In order for us to take the best possible care of your child during the day, please make sure that the nurse on your child's campus has the following items: 1. Up to date shot record, or an exemption form completed & notarized 👉 All new students must have this before the first day of school 👉 All incoming 7th graders must have Tdap and Meningitis (MCV4) 2. Doctor orders and action plans if your child has a health issue, such as Diabetes, Seizures, Asthma, etc. 👉 Orders must be renewed every year & signed by a doctor 👉 Action Plans must also be revised/updated every year & signed by doctor, parent, nurse 👉 Current and viable contact phone numbers are needed 👉 A meeting with your child's campus nurse may give you extra peace of mind 3. All medications that your child may take during school hours 👉 Must be in original bottle 👉 Must be labeled (correct student name, medication, dose, and when to administer) 👉 Authorization forms must be completed and signed for each medication 👉 All medical supplies your child might use during the school day If you have any questions or concerns, please contact your child's campus nurse.
